Not...entirely. I think Sasuke is something of a blind spot for Itachi because Itachi loves him so much. It's not always easy for him to see him objectively, and as such he's not always aware of Sasuke's feelings for him. In canon, one of his most emotionally honest moments is him talking to Sasuke not long before things go to hell and essentially saying that he understands that Sasuke resents him and that's okay because ninjas are meant to be hated and he'll always be there for him, anyway, even if it's just as an obstacle. And Sasuke spends the conversation totally confused and dismayed. So it's like, gj, missing how much your little brother adores you, Itachi. Not that he was totally unaware that his brother admired him but he just seemed much more aware of the negative side of Sasuke's feelings rather than how much Sasuke loved him. Though that is probably also related to Itachi resigning himself to being hated, but it's still a clear misinterpretation of Sasuke's feelings for him.

So anyway, in Aather! He's more aware and accepting of the fact that Sasuke cares for him and worries about him. And he understands that Sasuke is very attached to him right now and that he's...kind of attracted to him. But! Itachi isn't sure it's not just a phase brought on by the trauma of certain memories and recent events in Aather. He thinks there's a good chance Sasuke will eventually move on once he's moved past the trauma, and Itachi does try to subtly encourage Sasuke to not cut people like Naruto and Sakura out of his life because he does not actually see himself as being very good for Sasuke overall. But he's very aware that his betrayal and manipulation and abandonment of Sasuke is why he's so messed up now, so he is pretty determined to be there for him in whatever way he needs for as long as he needs it. So he's not going to do push him away "for his own good" or anything because he thinks that's a conclusion Sasuke needs to come to himself. And he's pretty sure he'll come to it eventually.

So, no, he does not totally understand how deep Sasuke is in it. :x His own feelings are complicated and not as dissimilar as he might like to claim. He is building his life around Sasuke in a lot of ways and is pretty preoccupied with him and being there for him and things like that can complicate feelings in unhealthy ways, especially when coupled with the stress and trauma games can cause. But Itachi is very good at ignoring his feelings and focusing on what the bigger priority is. And Sasuke is always the bigger priority. So he's much more focused on his brother than himself right now.
